Play All Day Waiver

Waiver of Liability and Indemnification

In consideration of the permission granted for my child(ren) to participate in activities at Play All Day LA, I, the undersigned, on behalf of myself, my child(ren), and any personal representatives, heirs, and assigns, do hereby agree to the following:

1. Assumption of Risk
   I acknowledge that playing at Play All Day LA involves inherent risks, including but not limited to physical injury, bumps, bruises, or accidents. I voluntarily assume all risks associated with my child(ren)’s participation, whether caused by negligence or otherwise
2. Release of Liability
   I, on behalf of myself, my child(ren), and any personal representatives, heirs, and assigns, hereby release, discharge, and covenant not to sue Play All Day LA, its owners, officers, employees, and agents from all liability, claims, demands, losses, or damages resulting from participation in activities at the playground.
3. Indemnification
   I agree to indemnify and hold harmless Play All Day LA, its owners, officers, employees, and agents against any and all claims brought against them for any reason, including but not limited to costs, expenses, and legal fees
4. Supervision and Conduct
   I understand that it is my responsibility to supervise my child(ren) at all times while at Play All Day LA. I will ensure that my child(ren) follow all safety rules and instructions. I acknowledge that if my child(ren) cause harm to another participant or to the facility, I may
   be held financially responsible
   
5. Medical Treatment Authorization
   In the event of an injury, I authorize Play All Day LA to provide emergency medical treatment deemed necessary by qualified personnel, and I agree to bear any costs associated with such treatment.
