Play All Day Waiver

In consideration of the permission granted for my child(ren) to participate in activities at Play All Day LA, I, the undersigned, on behalf of myself, my child(ren), and any personal representatives, heirs, and assigns, do hereby agree to the following:

1. Assumption of Risk
   I acknowledge and understand that there are known and unknown risks associated with participation in Play All Day playground activities and the use of the play area, any and all other Play All Day playground equipment, including but not limited to the Open play, which include but are not limited to: contusions, fractures, scrapes, cuts, bumps, paralysis, or death.
2. Release & Waiver of Liability
   I, for myself, the Participant(s) named, our heirs, assigns, representatives, and next of kin agree to hold harmless, release, waive and indemnify the independent owner of this Play All Day playground facility and employees from any and all injuries, liabilities or damages from participation, except for those arising from the gross negligence or willful misconduct of Play All Day playground.
3. Indemnification
   I agree to indemnify the independent owner of this Play All Day playground facility and employees for any defense cost or expense arising from all claims, injuries, liabilities or damages arising from participation, except for those arising from the gross negligence or willful misconduct of Play All Day playground
4. Supervision and Conduct
   I understand that it is my responsibility to supervise my child(ren) at all times while at Play All Day LA. I will ensure that my child(ren) follow all safety rules and instructions. I acknowledge that if my child(ren) cause harm to another participant or to the facility, I may
   be held financially responsible
   
5. Rules & Safety Requirements
   I agree that the Participant(s) named, and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any play station and/or any other open play event at Play All Day playground.
6. Emergency Medical Treatment Authorization
   I confirm that the participant(s) are physically able to participate. In the event of an injury, illness, or emergency, I authorize Play All Day staff to contact emergency services (911) and notify the parent/guardians contact listed on this form. I understand that Play All Day does not provide medical treatment, and that any medical care, transportation, or related expenses are responsibility of the parent/guardian.
7. Severability
   If any provision of this agreement is found invalid or unenforceable, the remaining provisions shall remain in full force and effect.
